Hypertension (Blood Pressure Management) Workshop Series- These workshops provide information to help control blood pressure and encourage participants to make healthy lifestyle choices and is designed to help patients improve their Self-Managed Blood Pressure (SMBP). Incentives are provided for participant commitment to the 12-week program in addition to the orientation.

Hypertension (Blood Pressure Management) Tracking- Learn more about managing and controlling your high blood pressure through our Hypertension Tracking Program in cooperation with UT Health East Texas Clinics. This program provides a one-on-one check-in with a designated Community Health Worker (CHW) at your clinic over the course of 12 weeks.
UT Health Tyler- Lifestyle Change Programs
Prevent T2 Program- This National Diabetes Prevention Program is approved by the Center for Disease Control (CDC) and taught by a CDC certified Lifestyle Change Coach. It is a year-long workshop series that is proven to help people with prediabetes prevent or delay development of type 2 diabetes by 58 percent.
